Northern Home Care provides
nursing care, home health aides, personal care aides and homemakers, as well as
health education and skills training. Agency services are provided on an
hourly, daily or live-in basis and Northern Home Care staff is available 24
hours a day, 7 days a week. Presentations are available to educate caregivers
and potential caregivers about the home care industry. Northern’s
goal is to empower individuals to make informed decisions about the care of their
loved ones and to minimize crisis.
Prior to the onset of service, a
professional nurse assesses each client to determine the level of care
required, and to evaluate any risk factors. A client can be assessed at no
charge or obligation in his/her home or in an institution. Simultaneously, all
payer sources are contacted to discuss the recommended care plan and to ensure
for the most cost effective use of coverage.
After the initial assessment, a care
plan is developed in conjunction with the Physician, the
patient, family/caregivers, case managers, and the payer source. Patient care
is monitored daily either through direct supervision, phone contact, or review
of records and reports. Information is communicated to physicians, case
managers, family members, and Northern staff to ensure for proper levels of
care and quality assurance.
Northern recognizes that
each person who receives our home care services, is
part of a larger network of formal and informal services. We focus not only on
the person who is receiving direct services, but on the Caregivers who have
needs for information and education about the aging process, skills training
regarding care of their loved ones with specific illnesses and disabilities,
and the need for peer support and mutual aid from others who are in similar
situations.
Northern Home Care accepts
Managed Care, Commercial/private insurance, and self-pay.
